Plan rates are guaranteed through June 30, 2008 for all groups with effective between 7/1/06 — 6/15/08.

Upon review of the information, all groups will receive a Medical Underwriting Rating Factor (MURF). This factor will range from .85 — 1.55 and will apply to standard rates.

All groups will have a $20 monthly administration fees. This fee is shown seperately from you medical rates.
